About school health checkups

Our clinic accepts follow-up examinations for school health checkups.
If your child takes the form home,
I recommend you have your eyes examined by an ophthalmologist.
Please feel free to contact us for advice.

If this is your first visit to our hospital, you will need to make an appointment.
Even if you have previously received a medical examination, you may need to make an appointment in advance depending on the items included in the school health checkup.
Please Contact us before coming to the clinic.

If you have any other symptoms that concern you, please feel free to contact us.
*Please bring your My Number health insurance card or eligibility certificate when you visit our clinic.

Reception hours

10:00 AM - 12:30 PM / 2:20 PM - 6:00 PM
If you are using contact lenses for the first time, please come at least 30 minutes before the above reception hours.
*Depending on the circumstances, a contact lens prescription (instruction sheet) may not be issued.
